What type of identification do you require?

We may ask for one or more of the following documents to verify your identity:

• Photo Identification
• Social Security Number (SSN) Card (we may be able to assist you even if you do not have SSN number)
• Mail/utility bill with name and address
• Any other document that shows your name and current address
